The woman that played the Twilight Zone role was Donna Douglas, Elly Mae from The Beverly Hillbillies. Per IMDB she did win a few beauty contests, but not Miss America.

This reminds me of a Twilight Zone episode. This woman was bandaged and they kept saying how ugly she was and how they were trying their hardest to make her pretty and "normal" - then it turned out that all the doctors and nurses were super deformed and she was gorgeous. (I think Miss America actually played the role.)
